Human errors account for an alarming rate of failures throughout a
broad range of industries worldwide. With high failure rates and the
fact that the results of human error can result in enormously costly
failures, including the loss of life, the need for human error
analysis and safety assessment is crucial. Methodologies for
analysing human error have been developed and adopted in several
industries, with NASA being a leader in this effort. Relex Software
and NASA have worked jointly to develop a software tool based on the
NASA adopted methodology to aid in the analysis of human error and
corresponding risk assessment. The resulting Relex ® Human Factors
Risk Analysis provides a comprehensive, systematic, and streamlined
approach to addressing this critical issue.
Proven methodology.
Relex Human Factors Risk Analysis is based on a
HF-PFMEA (Human Factors Process Failure Mode and Effects Analysis)
approach. HF-PFMEAs can be used to assess human safety and human
reliability by analysing human processes or tasks and are a proven
approach to reducing the frequency and severity of human error.
Risk analytics. Analysis begins with definition of process steps and
the possible errors which could occur. The analyst must then
determine the effects of those errors, as well as the associated
severity and likelihood. Providing a framework for this analysis,
Relex Human Factors Risk Analysis computes a Risk Assessment Code
(RAC), or risk score, for each error. By colour-coding the RAC
results, instant feedback is provided to identify problem areas so
that risk reduction techniques can then be effectively employed.
